The University of West Alabama Athletic Training & Sports Medicine Center and Andrews Sports Medicine & Orthopaedic Center will be conducting free pre-participation physical examinations for area secondary school athletes on Saturday, July 21st from 8:00 to 11:00 a.m. in Bibb Graves Hall.
Teams or individuals may be scheduled for physical examinations on this date by returning the form at
http://at.uwa.edu/Forms/HSPEform12.doc prior to July 16, 2012. Please note that this will be the only time this year that we will be providing pre-participation physical examinations for area high school athletes. Patients may also be scheduled in advance to be seen by the physicians at this time as well.
It is essential that each athlete desiring a pre-participation physical examination bring a medical history/physical examination form signed by their parents/guardian. Without this signed form we will not be able to provide a physical examination. For questions or concerns, please contact
